Operator · for allied health clinics doing $1M to $3M
Know what is actually happening across revenue, payroll, clinician performance and
risk. Then change what the numbers say to change. One operating constraint, one implementation
cycle, one finished asset every month. Operator is built for the owner who wants each year of growth
to leave the business stronger and their own week lighter.
Ten places in the founding group. Kicks off Tuesday 13 October 2026.
Watch: inside the program — 7 minutes
A short questionnaire, then a conversation. Shane Gunaratnam reads every application himself.
The problem at this size
It is very easy for growth to go the other way: more revenue, more staff, more of the owner. The next Health Professionals award rise lands on 1 October. And the honest answer to "what does each clinician earn the business, after what they cost to employ?" lives in a spreadsheet you built at midnight, if it exists at all.
Fit
The plan
Operator is a twelve-month implementation and operating program for established clinics. We build your commercial control layer for you from your own records. Then, every month, you and a small group of owners take one real constraint in your clinic and finish the work on it. The point is a clinic that is easier to run, not just a set of better reports.
The three rooms are not extra teaching. They are there so the decision you made in Week 1 is still alive in Week 4 and actually finished.
Across the year, the monthly constraint moves through economics, pricing, utilisation, payer mix, pay structures, recruitment, clinician ramp-up, retention, marketing, operations, leadership and your own capacity as the owner.
Don't take our word for it
"What resonated with Shane was that it was more about people and culture than just the numbers. We went from 45–50K a month to sitting between 55 and 60. It's also given me the confidence to raise my prices and sleep at night."
Frank Romeo — Director, Form Focus Physio
"Shane had literally done what I was trying to do. My own ego almost stopped me — I figured I could do this myself. In four months we went from the early fifties to just under $70K a month. Worth every single cent and then some."
Ryan Taylor — Director, Glide Physio
"My practice has grown from a solo operation to a small team of five. When I have felt out of my depth, he has helped me navigate the issues with a personalised and collaborative approach. Shane's mentoring has been invaluable to my personal growth in leadership."
Samantha King — Physiotherapist, True Balance Physio
Clients of the firm's private advisory practice. Operator is built from that work and runs it as a group program.
Apply for a founding placeYour first visible result
The first month is not a warm-up. It gives you a clear picture of your clinic that you can act on.
You send payroll, billings, contracted hours, contracts and your fee schedule. We connect them and build your commercial control layer. You do not build anything. Your dashboard is live within seven days of your complete data arriving.
By day thirty you have five things:
From there you join the monthly cycle wherever the group is, and your own plan runs alongside it.
Apply for a founding placeWhat is included
Payroll, billings, diary and contracts connected, with your dashboard built and maintained for the whole twelve months.
Why it matters: you get the answers without becoming a data person, and your time goes into decisions instead of spreadsheets.
Live with Shane. The month's constraint worked on members' own numbers, ending in decisions and a started deliverable.
Why it matters: each month starts with a decision made on evidence, not a topic to think about.
Live with Shane, three weeks in every four, recorded.
Why it matters: you hear how other owners' implementations are going, and yours gets unblocked before it stalls.
Between sessions, post implementation questions in the Operator space. They are answered with Culture of One's engine and your own numbers behind them, and the rest of the group can see and add to them. Anything that needs a judgement call is brought to the next Operator Room and worked through there.
Why it matters: small implementation questions do not wait a week, and the bigger calls get the whole room.
Each week, a short note showing which clinicians are on track and which need attention.
Why it matters: you start the week knowing where to look.
Classifications and forward wage-cost figures are sourced to the Health Professionals and Support Services Award.
Why it matters: you can show your accountant, bookkeeper or a staff member where every figure came from.
Circle for the workshops, rooms, recordings, assets and the group.
Why it matters: nobody has to go looking.
The structure is shared; the work is not. Every month's work is done on your own payroll, billings and diary, and it is not finished until the asset exists inside your clinic.
Why it matters: you finish the year with assets in your business, not modules you watched.
Proof, from real clinics
The work behind Operator already runs on real allied health clinics' payroll, billings and operating decisions. Some of what it has found, with every practice anonymised:
The same clinician cost-to-billings calculation has been built from the payroll and billings of several practices and reconciled line by line. Their results are far apart, which is why an industry average cannot tell you where your own clinic stands.
Apply for a founding placeWho runs it
Founder of Culture of One. Physiotherapist since 2008, and formerly managing partner and practice principal of an allied health clinic. Operator is how established clinic owners now work with Culture of One — Private Advisory is at capacity and closed to new clients.
"Shane has a way of cutting through the noise and giving you exactly what you need to move forward in your business. I've done other coaching programs, but this one stood out."

How participation works
Your part and ours. You supply accurate information, make the decisions and implement them in your clinic. We build and maintain the infrastructure and work each decision with you. We do not run your practice or manage your team. Operator is a group program with a set monthly structure, not a course library and not private one-to-one Advisory.

Before you apply
How it works
A few minutes. Your clinic's size, your team, and what you want the business to do that it isn't doing now — in your own words.
Every application, personally. No sales team, no triage bot.
A short call that starts from what you wrote, not from scratch. You ask your questions, he asks his.
The conversation is there to protect the quality of the room, and the room is ten owners. That is the whole founding group.
And pointed at the thing that is — whatever your size. The application exists so that conversation happens before any money does.
